In this role, you will:
- Communicate with patients monthly via telephone and update pertinent health status changes in OncoEMR
- Review and reconcile medications on a monthly basis
- Escalate symptomatic patients to Triage for further assessment
- Collaborate with careteam to help ensure positive patient outcomes
- Maintain accurate and clinically appropriate documentation
- Provide patient education with respect to medications and disease management
- Adhere to HIPAA guidelines while handling sensitive patient information
- Current NYS LPN license required
- Consistent professional conduct and meticulous attention to detail
- Excellent verbal and written communication skills as well as excellent interpersonal skills with
- patients, staff, and other healthcare professionals
- Bi-Lingual, English/Spanish a plus
- HemOnc experience preferred
