In this role, you will:

Communicate with patients monthly via telephone and update pertinent health status changes in OncoEMR
Review and reconcile medications on a monthly basis
Escalate symptomatic patients to Triage for further assessment
Collaborate with careteam to help ensure positive patient outcomes
Maintain accurate and clinically appropriate documentation
Provide patient education with respect to medications and disease management
Adhere to HIPAA guidelines while handling sensitive patient information

We require:

Current NYS LPN license required
Consistent professional conduct and meticulous attention to detail
Excellent verbal and written communication skills as well as excellent interpersonal skills with
patients, staff, and other healthcare professionals
Bi-Lingual, English/Spanish a plus
HemOnc experience preferred

Salary up to $33/hr and benefits including Tuition Reimbursement, Health Insurance on day 1 of employment, Dental, Vision, Life Insurance, Short and Long term disability, 401k Plan, generous PTO time, 8 paid holidays (2 floating) and more.
